There are a lot of reasons why it could be.
It could be because Sean Punch doesn't like to stick with pure standard iconography when depicting his monsters.
It could be because flails are pretty damn effective in GURPS.
It could be be because pitchforks have poor armor penetration.
Or it could be because of three words:

Dio

You know how people will talk about politicians being able to look at you as if you're the only other person in the room? (Bill Clinton and Ronald Reagan spring to mind as ones I've heard that said about.) Ronnie James Dio had that. I saw him twice in the late 1990s and I swear he store straight at me in the back of the venue.
